Denmark Family Practice can provide individualised care plans for those with any chronic disease. Chronic Disease Care Plans (also known as GP Management Plans) are a benefit to patients and can be provided by your GP. 

 

Your doctor can prepare a specific review of your health needs and together you set goals and action plans to help you achieve the best quality of life.  

 

Your Care Plan can assist you in accessing medicare benefits to reduce the cost of allied health visits (e.g. physiotherapy and clinical psychology).

Your doctor can arrange a specialised team of allied health care providers (where appropriate) and will liaise with them to ensure the best care. Quite often, a medication review with a pharmacist is conducted and your care plan will be reviewed to ensure it's still well suited to your needs. 

  

Routine health care assessments are arranged for particular age groups (special medicare rebates are available):

• 4 years

• 45-49 years

• Over 75 years of age

Patients with Asthma and Diabetes can access specialised care plans including spirometry testing and lung function tests in asthma and ongoing monitoring of diabetes to prevent complications.
